| This site is a full-featured ordering system for your convenience and security. Utilizing the ShopSite "shopping cart" system, you may browse through the forms available on this site and select the products you wish to purchase in a virtual shopping cart. All personal information is protected from prying eyes through data encryption and secure-server technology. We will never use or distribute your personal information in any way except to send you your order. When you've completed your selections, just choose "View Cart" from any of the pages, and you can view the forms you've selected, add or subtract them from your order, or edit the quantity of each product. Once satisfied, click the checkout button and enter your payment information. For your security, convenience, and comfort, there are several payment options:

Secure, on-line payment via PayPal: At checkout, you will be directed to PayPal which will provide options for secure payment by either the credit card you prefer (VISA, Mastercard, Discover, & AMEX), or through your PayPal account.
Once successfully ordered, you will immediately receive an e-mail containing the links to download your purchased forms. To download the forms you recently ordered, click the URL link provided in the e-mail for each item (or copy and paste the entire URL address into your browser's address field). Once opened or downloaded to your browser, you may save the forms to your computer. You will have up to 10 days to download the forms you have ordered, with a total of 10 retries.
